Pathway Family Services: How Enhanced Family Services Can Help You

Welcome to Pathway Family Services Enhanced Family Services (Care Management), where we provide additional support and services to Medi-Cal members facing complex health challenges. Our goal is to assist individuals in overcoming obstacles that hinder their ability to improve their health, including external factors such as housing instability.

About Enhanced Family Services

Enhanced Family Services offers seven types of complimentary services designed to enhance your health and well-being. As a qualified participant, you’ll have access to a dedicated care team led by an ECM Lead Care Manager, all provided as part of your current insurance provider Medi-Cal plan. Your existing Medi-Cal benefits remain unchanged, ensuring continuity of care with your current healthcare providers.

Seven Ways Enhanced Family Services Works for You

1. Help to Stay Engaged in Your Care

Your ECM Lead Care Manager and care team will ensure you remain focused on your health by providing necessary services and support tailored to your needs. We meet you where you are, whether at home or during appointments.

2. Help to Connect You to Community and Social Services

Beyond healthcare, ECM helps you access non-health services such as food assistance, job training, childcare, and disability-related support. Your care team assists in identifying and connecting you with relevant community resources to help you maintain independence and stability.

3. Help to Craft a Plan

Collaborate with your care team to create a personalized care plan covering various aspects, including:
 

  • Doctors you visit
  • Health goals you set
  • Services you receive
  • Physical, behavioral, oral health, and substance use treatment needs
  • In-home services and neighborhood/social services

Your Lead Care Manager keeps your healthcare providers informed about your health status and services received. Additionally, they assist you in:

  • Applying for supportive services such as meal delivery, housing, and personal care
